It might be good to require approval for new members who post more than $totalLinksPerMessage links in a message, before they reach a total of $limitCommentsForNewMembers comments.
In that case "New members" are defined by the total number of comments they have made on the board compared to $nbCommentsForNewMembers.
Suggestion :
$totalLinksPerMessage = 1
$nbCommentsForNewMembers = 5

Step 3: A numeric code (or password) in made available to them.
Step 4: They must enter that passcode/password to join the forum.

Also we can ask a custom question like "What's the latest version of Gideros?" while registering. (btw, I don't know if these spammers are usually bots or regular people.)
I'm pretty sure some forums have a limit of links for the first comments.
That's a good solution (the best) between approving users manually (takes so much time) and letting the doors open.

I agree that the spam problem is not easy to solve but instead of coming up with xx custom solutions there are already a few solutions provided so you could cut 75% (arbitrary chosen ) of your spam without losing to much resources on it.
One of the most efficient automated solution I've seen is this : "In Vanilla, it’s possible to prevent members with a low Rank from posting links, editing comments, etc."
You can achieve good results by playing with it (which is related to my first suggestion at the top of this discussion).

Well, I'm not discussing if it's a good solution or not : I just know from experience it is.
That's why I said : "You can achieve good results by playing with it" until you find the sweet spot.
